Office visits are by appointment only. Please call the office to schedule an appoinment.
Cancellations/Missed Appointments
There is a $75.00 fee for cancellations with less than a 24-hour notice and for missed appointments.
Emergency & Urgent Calls
For situations that are an emergency, please call 911 or go to the nearest emergency room. For urgent matters, please text me at (714) 942-5552 or through the Spruce app.
Prescription Refills
Please call your pharmacy and have them send a refill request to the office. The pharmacy can do this either electronically or via fax at (714) 256-9013.
Forms
You will be charged up to $50.00 for disability and other forms to be completed depending on the length and time involved for each form.
